Upon making an appt, you will receive information to make your own secure patient portal where you will have access to the client information and history forms, insurance and financial authorizations and other intake forms. If you have difficulty with completing them online, I can mail them to you but be aware it may delay scheduling your initial appointment until I receive these completed documents.
